RESUMEN

The objective of this systematic review is to determine the association between interstitial lung diseases and chronic periodontitis from various aspects such as microbial, biomarker, genetic, and environmental levels. A systematic review was carried out from 2000 to 2021 following Preferred Reporting Items for Systematic Reviews and Meta-Analyses (PRISMA) recommendations including studies searched in PubMed-Medline, Google Scholar, and Cochrane databases. A total of more than 100 articles were obtained in the initial screening process. Out of these 42 studies fulfilled the inclusion criteria and were included in the study. According to the extracted data, there is mounting evidence suggesting the association between these two diseases. Our systematic review raises the prospect of a connection between chronic periodontitis and interstitial lung diseases, within the limitations of the studies we included.

Periodontal disease is associated with many systemic diseases, such as cardiovascular diseases, atherosclerosis, diabetes mellitus, stroke, and pulmonary diseases. Interestingly, recent literature suggests that periodontal disease might be a risk factor for various cancers such as lung, colon, oesophageal, head, and neck cancers. However, the precise mechanistic link is lacking. Hence, in this meta-analysis, we aimed to investigate the correlation between periodontal disease and lung cancer in periodontally diseased patients. Data were searched for relevant studies from 2010 to 2022. We correlated periodontal disease and lung cancer based on adjusted ORs/HRs and associated CIs. I2 statistic was used to assess statistical heterogeneity. Publication bias was analyzed by visually inspecting the symmetry of the funnel plot and Egger's test. The study is registered in the International Prospective Register of Systematic Reviews (PROSPERO; registration no: CRD42023390819). A total of 194,850 participants from observational studies (two case-control studies and five cohort studies) were incorporated for the current analysis. The meta-analysis of included studies showed an overall effect size (risk ratio) of the periodontal disease group with respect to the non-periodontal disease group for lung neoplasm to be 1.41 (95% CI: 1.32-1.52). The value was more than 1, indicating that the periodontal disease group had a relatively higher lung cancer prevalence than the non-periodontal disease group. Further, the overall risk ratio was found to be statistically significant (p<0.00001). Moreover, the funnel plot suggested some degree of publication bias. Evidence in our study implicated that there is an increased risk of occurrence of lung cancer in chronic periodontitis patients.

BACKGROUND: This study aimed to evaluate the efficacy of local drug delivery system of zoledronate (ZLN) gel as an adjunct to scaling and root planing (SRP) for the treatment of human periodontal intrabony defects clinically and radiographically. MATERIALS AND METHODS: Forty intrabony defects (three walled and combined defects without involving furcation) in moderate to severely affected forty chronic periodontitis patients (range, 30-50 years) were randomly divided into two groups and treated either with 0.05% ZLN gel (ZLN n = 20; 1 dropout) or placebo gel (control group [CG] n = 20) after SRP. Clinical parameters such as plaque index (PI), gingival index (GI), tooth-specific pocket probing depth (Ts PPD), and clinical attachment levels (TsCAL) were assessed at baseline and at 3 and 6 months using occlusal acrylic stent. Radiographic parameters were assessed at baseline and 6 months, utilizing "ONIS 2.5 PROFESSIONAL" and "SYNGO" software compatible with DentaScan to measure the volumetric bone changes in intrabony defects. RESULTS: In intragroup comparisons, both groups showed significant PI and GI reduction (P < 0.001) after treatment at 3 and 6 months. In intergroup comparisons, Ts PPD reduction and Ts CAL gain were significant only in ZLN at 6 months from both baseline and 3 months. Radiographically, significant reduction in defect depth and buccolingual width with volumetric defect gain of 40.24% ± 7.44% in ZLN compared to insignificant gain of 1.60% ± 4.06% in CG was observed at 6 months. CONCLUSION: ZLN gel applied subgingivally in intrabony defects resulted in significant improvements both clinically and radiographically.

PURPOSE: To determine whether a fixed partial denture (FPD) or an implant replacement of a single missing tooth leads to better masticatory efficiency and patient satisfaction. MATERIALS AND METHODS: One-hundred and twenty participants with missing mandibular right first molars were selected on the basis of predefined inclusion and exclusion criteria. After obtaining informed consent, 60 participants were randomized to the teeth-supported (FPD) group and 60 to the implant placement group (IMP). The study was divided into the following parts: (i) Completion of a Likert scale satisfaction questionnaire (postrehabilitation) by the participants. (ii) Evaluation of masticatory efficiency and performance 3 months after rehabilitation. Data were evaluated by applying t-test and z-test using statistical analysis (α ˂ 0.05). RESULTS: Masticatory efficiency of participants in the IMP and FPD groups was 74.95 ± 0.90% and 74.41 ± 3.35%, respectively (p = 0.607). Total satisfaction questionnaire scores for the two groups were also not significantly different; however, the mean scores of overall satisfaction and function categories in this questionnaire were significantly higher for the IMP group, while the mean score for the duration of treatment question was higher for the FPD group (p < 0.05). CONCLUSION: The results of this study suggested that although masticatory performance and efficiency were not statistically different for single teeth replaced with implants or FPDs, patients perceived higher satisfaction with implant restorations; they also preferred the shorter treatment times for rehabilitation in the FPD group.

RESUMEN

Close eruption technique is preferred in deep, buried, intraosseous and labially impacted teeth to provide healthy and fuinctional attached gingiva but in this technique failure of bonded attachment usually means, repeat exposure of the impacted tooth. This article describes an innovative method for bonding two attachments (Double Bonding System) in combination instead of one. It provides the safe and determinate system for ortho-eruption, avoid the trauma of patient from re-exposure and enhance the comfort as well the confidence of the operator.

Rosai-Dorfman disease is rare. When it does occur, it usually affects children, and it has a propensity for whites and blacks as opposed to members of other races. We report the case of a 45-year-old Asian Indian woman who presented with painful masses in the area of the axillary, cervical, and inguinal lymph nodes. She had a decade-long history of tonic-clonic seizures and a recent history of digestive complaints and progressive nasal stuffiness. Endoscopic examination of the left nasal cavity revealed the presence of submucosal bulges along the septum and the lateral wall; hypertrophy of the adenoids was also noted. Histologic analysis of lymph node specimens revealed dilated parenchymal sinuses, germinal activity with infiltration of numerous histiocytes (emperipolesis), and chronic inflammatory cells. The patient was diagnosed with Rosai-Dorfman disease (sinus histiocytosis with massive lymphadenopathy) with atypical extranodal involvement (the nasal area). An aggressive course of polychemotherapy thrice daily was initiated, but it had no lasting effect, and the patient died 8 months later of an undetermined cause. Our patient's age at the onset of her disease, her race, and the extranodal involvement make this case of Rosai-Dorfman disease unusual and perhaps unique.
